Event speaker logistics form
Your speakers are confirmed, and now the team needs to know when they arrive, what they need and who looks after them.
The short answer
A speaker logistics form collects only what the team will act on: arrival and departure, travel and stay needs the organiser has agreed to arrange, access or dietary needs, technical needs and one contact for the day.
Send it once, with a reply date, and carry the answers into the speaker briefing generator so the speaker sees one consistent pack.
Collect less, act on all of it
Every field on the form should have an owner who will do something with the answer. If nobody will, remove the field.
Check what the organiser has actually agreed to provide before asking. Asking about a hotel when none is offered creates an expectation you then have to manage.

Sensitive information needs a reason and a limit
Health, dietary and access needs are personal information. Say why you are asking, who will see it and when it will be deleted, as far as your organiser has decided.
Ask your data protection owner how the form should be worded and stored. Collect identity document details only if a venue, airline or authority requires them and a competent party has told you so.
Sequence for sending and chasing
- Confirm what the organiser will provide for each speaker before drafting the form.
- Send the form with the session brief, with a reply date that sits before ticket or room deadlines.
- Chase once by a different channel than the first message.
- Merge replies into one sheet and mark unanswered fields as open, not assumed.
- Send each speaker a short summary to confirm, then lock the sheet and note who changes it afterwards.

Eight speakers, three open fields

A fictional chartered institute has eight speakers for a one-day conference. Five return the form on time. Two forget the departure time. One has not said whether she needs a lunch adjustment.
The liaison calls the three, records the answers with the date, and leaves the lunch question marked open until the speaker replies, so the catering lead does not guess.

- Speakers are mostly local and have simple needs.
- The secretariat has handled the same form before.

- Speakers arriving from several places with travel booked through different parties.
- Accessibility and dietary needs spread across venue, catering and transport suppliers.
- Nobody on the team has time to chase replies and merge the sheet.
